Appraisal of Relationship between Hypertension and Short-Term Heart Rate Variability Indices

Background: Hypertension, the silent killer is as an important risk factor for cardiac arrest, stroke, kidney disease and increased mortality rates in adults. Heart Rate Variability (HRV) is a physiological phenomenon of variation in the time interval between heartbeats and is a gold-standard to assess the perpetually changing oscillations of a salubrious heart. Early detection of hypertension by a non-invasive tool as HRV can identify the autonomic regulation in hypertensive individuals at high risk for developing cardiovascular morbidities complications.  Aim: To study the autonomic correlates of cardiovascular health in hypertension in terms of short term HRV and to analyze the association between HRV indices and elevated blood pressure. Methods: It was a cross-sectional observational study carried out in Sports Physiology Laboratory in Department of Physiology of a rural medical college in central India. 25 known cases of hypertension (having Systolic BP >140 mmHg and diastolic BP >90 mmHg) in the age group of 35-60 yrs and 75 age matched normotensives as controls (Systolic BP Results: The mean age of control males was 39.80±6.68 and mean age of control females was 37.05±2.75. Out of hypertensives, 17 were males (mean age 42.58 ± 6.94) and 8 were females (mean age 42.6±8.84). There was statistically significant difference observed in SDNN, RMSSD, HF%, HF nu between control and cases males but no significant relationship was observed among other parameters in males. On comparing the sympatho-vagal ratio (LF/HF), female cases were having high ratio than control females indicating a dominant sympathetic response. From Pearson’s correlation analysis, a positive non-significant relationship was observed between systolic BP and Frequency domain indices in controls while in cases a negative correlation was found which was also statistically non-significant. A strong positive correlation was found between Systolic BP and SDNN (r=0.55) in hypertensives. The time domain parameters were also found to be negatively correlated with mean heart rate in both controls and cases. Conclusions: This pioneering study analyzed the association between heart rate variability indices and high blood pressure.  The analysis of HRV can distinguish parasympathetic from sympathetic influences on the heart and provides important insights into the role of the autonomic nervous system in the pathogenesis of essential hypertension. HRV is reduced in men and women with systemic hypertension. Among normotensive men, lower HRV was associated with greater risk for developing hypertension. These findings clearly exhibit that autonomic dysregulation is present in the early stage of hypertension.

Decoding the Intricate Nexus Between Hypertension and Short-Term Heart Rate Variability: A Narrative Review

Hypertension is among the most significant global health issues to date and is closely related to the autonomic nervous system, as evidenced by short-term heart rate variability (HRV). Short-term HRV is a noninvasive marker of the autonomic regulation of cardiac activity, related to the balance between sympathetic and parasympathetic activity. This is a narrative review on the complex interaction between hypertension and short-term HRV, considering pathophysiological mechanisms, clinical implications, and potential applications in diagnosis and management. Evidence has shown that hypertension is associated with reduced HRV, which means autonomic dysfunction and increased cardiovascular risk. The mechanistic pathways include alterations in increased sympathetic activity, and reduced vagal tone. Decreased HRV has also emerged as a predictor of the development and progression of hypertension, suggesting that it may be a useful biomarker. However, standardization of HRV measurement and interpretation are some of the challenges that have been identified. This review integrates the current knowledge, highlights gaps in understanding, and proposes directions for future research that may help leverage HRV in the management and risk stratification of hypertension.

Is High Flow Nasal Cannula Superior to Conventional Oxygen Therapies in COVID-19 Induced Hypoxia and other Type II Respiratory Failures

COVID-19 belongs to the severe acute respiratory syndrome (SARS) virus family, which includes virus strains that cause the common cold and flu, and directly affect the lungs by damaging the alveoli, thus impairing gaseous exchange. It creates a condition of silent hypoxia; although each person's symptoms of silent hypoxia may differ, the most typical ones are wheezing, sweating, confusion, and change of lips and skin colour. This silent hypoxia condition can potentially cause irreparable damage to vital organs if left untreated. So we need to oxygenate the patient at the earliest. The goal of oxygenation is to maintain the saturated partial pressure of oxygen (SpO2) level between 92-96%. For this, we used various assisted breathing devices (ABDs); the most used devices for covid-19 treatment were oxygen therapy devices, continuous positive air pressure (CPAP), bilevel positive airway pressure (BiPAP) devices and invasive ventilators. Where all conventional oxygen therapies are still effective, High Flow Nasal Cannula (HFNC) stands one step ahead and is a helpful aid during covid times. HFNC provides warm and humidified oxygen with a high flow rate of up to 60-80 L/min, compared to the 15 L/min flow of conventional oxygen therapies. Thus oxygenation not only got better, but the treatment also became simple and painless since it is a non-invasive technique (NIV) technique. In covid times, HFNC reached many small hospitals. Due to its increased availability, even more clinicians have access to it now. They are using it to treat patients with chronic obstructive pulmonary disease (COPD) and acute respiratory failure (ARF) and get good results. HFNC is becoming a better substitute for standard oxygen therapies and NIV to manage patients suffering from hypoxemic ARF. HFNC also has some drawbacks associated with it. However, despite its weaknesses, increasing evidence base advocates HFNC therapy as a new, advanced, state-of-the-art and efficient method for the initial management of patients with respiratory failure.

Evaluating Risk Factors for Adverse Nerve Outcomes in Monteggia Fractures: A Narrative review

Monteggia injuries are uncommon but involve severe elbow trauma with various injury patterns. Treatment approaches for these injuries remain contentious and vary significantly. This narrative review focuses on the posterior interosseous nerve in Monteggia injuries, aiming to elucidate the incidence of nerve injuries, assess nerve outcomes, and explore associated complications. We initially reviewed 642 publications and included 20 retrospective studies, which comprised 5 case reports and 15 randomized control trials. Patients were predominantly classified using the Bado classification system, with 83.66% having Bado type I fractures, 9.52% type II fractures, 6.10% type III fractures, and 0.72% type IV fractures. The average age of the patient population was 23 years. Common complications include ulna non-union and limited range of motion, while injuries to the medial nerve and the posterior interosseous nerve are among the most debilitating. Meta-analysis indicates a recovery period of 12-24 weeks following nerve corrective interventions. Further biomechanical and clinical research is needed to better understand the factors influencing nerve and associated recovery.

Automation of Mini Lathe for Production of Soldering Iron Bits

The increasing demand for automated devices in metalworking industries has driven significant advancements in lathe manufacturing. Conventional lathe operations, involving manual loading of copper and brass raw materials across three separate machines for taper turning, facing, and parting-off, have been observed to be inefficient. These manual processes result in excessive production time, increased labor costs, high raw material consumption due to human errors, and suboptimal product quality, thereby negatively impacting production capacity and overall cost-effectiveness. This study proposes an automated solution that integrates these three machining operations into a single automated mini lathe, enhancing efficiency without requiring a complete structural overhaul of traditional lathe machines. The implementation of automation has demonstrated a substantial improvement in productivity, increasing production capacity from 50 to 125 soldering iron bits per hour while optimizing space, time, and resource utilization. The results indicate that automation in lathe industries can significantly enhance manufacturing efficiency, reduce operational costs, and improve product quality.

Steroids in ARDS Management: A Review of Recent Advances and Clinical Guidelines

Acute Respiratory Distress Syndrome (ARDS) is one of the most daunting challenges in the field of critical care medicine, characterized by diffuse alveolar injury, severe hypoxemia, and high mortality rates despite the progress in mechanical ventilation and supportive care. The use of corticosteroids in the management of ARDS has been controversial in the past, with shifting attitudes being fueled by recent clinical trials, meta-analyses, and empirical data, especially with the COVID-19 pandemic. Corticosteroids are presumed to modulate the dysregulated host response that lies at the core of ARDS, given their anti-inflammatory and antifibrotic effects. This narrative review embarks on a critical review of the pathophysiologic rationale for the use of steroids in ARDS, reviews landmark clinical studies that have shaped current clinical practice and distils recent guidelines by major health organizations. The review also explores heterogeneities in steroid responsiveness based on the timing of administration, etiology of ARDS, and the unique phenotypes of individual patients. Through the synthesis of the latest evidence, this article seeks to provide clinicians with an integrated view of the judicious use of steroids in ARDS while promoting an evidence-based approach to improve patient outcomes.
